Listen for the exact word describing how someone feels or what they notice. Headache, fever and cough are different words, even though all can appear in a conversation about feeling unwell. Some descriptions use an adjective, such as dizzy or hoarse; others use a noun. Let the rest of the sentence help you hear which one it is. Then catch when the feeling started or what happened around it. Write the description as spoken without adding a reason the speaker did not give.
