Follow the result first, then listen for the earlier event that explains it. In this set, because often leads to had and a past participle. That earlier action gives the reason for a past feeling, appearance or difficulty. The result can have a past simple verb while its cause uses the past perfect, so the forms need not match. Keep the quiet had even when you already understand the cause. Write both parts as heard, including any negative that changes the result.
